I am currently a fourth-year medical student going into internal medicine. When someone asks me, what books/resources did you use to survive medical school, several key resources come to mind: USMLE First Aid for Step 1 (aka the med school bible), pathoma, sketchy micro, onlinemeded, netters, and U-World. But now there is a new kid on the block... One that I can comfortably say with 100% confidence stands on par with these giants and is a must read any learner/aspiring healthcare provider… and that book is “Frameworks of Internal Medicine” by Dr. Mansoor.

After, stating such a powerful claim, you are probably wondering how I can make such a statement? Let me explain:

1. One of the hardest aspects of being a physician is creating a differential diagnosis. When you approach a medical student and ask them to list out a differential, let''s say, pleural effusion, they immediately list out ten potential causes without a clear foundation in mind. Why is this a problem? Well to start, your list is likely to be incomplete without a basic structure. If I asked you the same question, but first asked you list all the causes of transudative pleural effusions followed by exudative pleural effusions, I guarantee your overall list will be longer. Your understanding of disease pathophysiology goes way up. If you listed nephrotic syndrome under a cause of transudative pleural effusion, it is easier to work out the physiology of pleural effusion secondary to nephrotic syndrome in your head if you already knew that it is a transudate process.
2. Approaching each patient with a basic framework results in less room for error, less likely to miss a diagnosis, and a decrease anchoring bias. When you approach the lung exam, the first step is to inspect the lung, looking for asymmetry, deformities, etc. You do not just jump to auscultation. If you made that mistake, you might miss the possibility that a patient has a winged scapula, decreased breath movement on one side, etc. This is why having an approach in mind is necessary. You can even make the same argument for any procedure. Creating a differential diagnosis is no different. Frameworks for Internal Medicine forces you to build these pathways in such an elegant way that your ability to nail that final diagnosis goes way up. You are not only able to list more possible diagnoses, but also think through the physiology of each one. That is the beauty of frameworks.

This book is superior in almost every way. The question/answer format will force you to stay engaged and learn. This book will change your approach medicine and build you a foundation that will have a strong positive impact on the rest of your career. A must read for any learner at any level who wants to have a systematic approach to how they see a diagnosis. A book that I believe should be in the hand of every student in the healthcare field.

I''m a new graduate NP working as a PCP at an FHQC. I bought this book to help me puzzle through cases that are too difficult for me to solve on my own (and with my mentors). Let me tell you about a patient whose life was changed because I own this book: I had one patient who presented at first with some dizziness and hearing loss- history of heavy drinking and SCLCa (currently untreated- but being monitored), neuropathy as a result of previous chemotherapy, and COPD. Over the course of the last 7 months, he had been getting progressively worse. Dizziness, diplopia, nausea, hearing loss, B/L weakness, unsteady gait. At the first visit, I ordered an MRIs to r/o metastasis to the brain. When this was negative, went on for further w/u: referred to neurology who came up with a + paraneoplastic lab which no one had any interpretations for (they told him to follow up in oncology). Oncology told me that this lab was insignificant and that the provider who ordered it should follow it up. (I was seeing patient for f/u between specialists). Now having B/L parasthesia, worsening balance (now using walker), ataxic gait, strabismus.... 4 months later with these worsening symptoms I do another MRI... no changes. Neuro thinks it is from the drinking. Oncology says negative for Ca. I send to ENT for the hearing loss- they confirm severe hearing loss without other suggestions. They send him back to me. Somewhere along the line I do a rheumo w/u and he has a +ANA : I''m not sure what it means in the context of his current symptoms and send him to rheumo on a hunch that this could perhaps be autoimmune & affecting neuro system: they say his ANA is positive from the cancer and no other suggestions there... He comes to see me again in the last 2 weeks- now wheelchair bound and unable to walk, slurred speech (at this point he has been to ENT x2, Neuro X3, Oncology X2, and Rheumo X1 and to me at least 5-6x). The family is beyond frustrated. I have no other ideas- I''ve talked to everyone else at my office for advice and not getting anywhere. I work through this book: And do you know what diagnosis I land on in the first 15 minutes of reading? Lambert-Eaton syndrome. Plain as day. Mind you: I have NEVER EVEN HEARD OF THIS DIAGNOSIS. I have never learned about it. It has never been casually mentioned at any lectures. At any rate, I send him to the ED and give report/summary that I am wondering if this is the cause.... He ended up getting immunoglobulins & sent for transfusion. I feel partially like a rock star, but it is 100% to the credit of this book. And the only thing I regret is that I did not use it sooner when I was trying to figure out what was going on. It''s amazing how user-friendly this book is: a new graduate NP can navigate a lot of information easily & figure out a diagnosis that many specialists with MUCH MORE experience have missed. The lay-out of the content works beautifully to be just the right amount of information for framing a problem without taking a ton of time in the minutia. I came to the diagnosis after looking at the book for <15 minutes (of course, I was not SURE it was right, but MAN: It really got me there). I use this as my "light weekend reading" to solve cases that I''m struggling with and so far it has been indispensable to my practice. Sometimes UpToDate has SO MUCH information that it''s hard to navigate/get a grasp on understanding some of the big concepts. I LOVE how this book connects and delineates pathophysiology by system/symptom & does not take a large time investment to see returns. It''s an amazing reference and has helped me (and my patients!) immensely and I think EVERY provider should have it. Not only because you will feel smarter & get respect for solving difficult cases- but because it makes a real difference in patient outcomes & really helps deepen your understanding. If only I had a photographic memory! I can not recommend this book enough!!

I am a current medical resident, and purchased this book to help review and master common internal medicine topics. The book''s approach is to the take the most common clinical presentations you will see on the wards and provide a structure to systematically approach each problem. As the title suggests, this "Frameworks" approach breaks each clinical presentation (e.g. "Chest Pain") into broad categories, then gradually builds increasing detail, in a structured approach that will help you learn as you read. In addition to visually presenting each framework as a decision tree, the text is written in a question-and-answer format that will force you to think through it as you go.

The book is superb, and has improved my approach and understanding of topics that I have previously been exposed to many times. As a resident, you have seen much of this information before, but the book presents it in a clear and systematic way that will help you master it and apply in on the wards. I have read a thousand ABGs, but doing the acid-base section definitely improved my ability to read and interpret them more quickly and accurately on rounds.

For medical students who are learning this for the first time, this book would be superior resource for learning and applying the material on board exams and on clinical clerkships. I would put it up there with Pathoma on my must purchase list of golden med school resources. A great read! I highly recommend!

This long awaited book is the perfect companion to both medical students and medical educators alike.

For students:
Shine on your medicine rotations as you build a fundamental "framework" approach to the most common complaints you will encounter in medicine. This book presents an easy way to organize a differential surrounding common complaints and provides high yield key points in bullet and question format. I really wish I had this resource as a medical student to quickly find an organized approach to different chief complaints. Now, as someone who helps educate students on medicine wards, I would be incredibly impressed by a student who is able to think through a differential in this organized manner that promotes long term retention.

For educators:
This is an incredible resource to reference when creating "chalk talks" on the medicine wards. Students tend to find these brief, high-yield, learning sessions very enjoyable and helpful. Having these "frameworks" to draw out really make for organized and effective teaching sessions. As an added perk, the book comes with a code to be able to quickly reference the frameworks on a mobile app.

Overall, this is a long overdue gift to the field of medicine and I foresee this book becoming a household name amongst medical trainees. It is much more user friendly than pocket medicine and really provides a solid organized knowledge base that is very applicable to the clinical setting while highlighting high yield points for exams. I reference this heavily when creating brief "chalk talks" for students on the wards and I cannot recommend this enough! Buy this book!

As an assistant professor of medicine and APD of our IM residency, I bought this book primarily to help me teach residents and medical students. It has been an outstanding reference to help me both expand differential diagnoses and (more importantly) to better organize my thoughts and approach for various problems. The unique style of this text eschews the traditional textbook model of "table 1, a list of causes of X" and instead methodically builds each diagnostic house: question by question, category by category, brick by brick. It mixes just enough pathophysiology to help ideas stick in the learner''s mind with a major emphasis on clinical bedside-ready knowledge. Everything about the style is intentionally and effectively designed to maximize the value of time reading; and each chapter feels like a teaching session from a master clinician-educator.

I highly recommend this book to all junior students interested in general medicine and especially to all interns and residents in internal medicine. It is the best thing a trainee can read to improve differential diagnosis skills and overall ability to solve clinical problems. I also recommend it frequently (along w Steven McGee''s "Evidence-Based Physical Diagnosis" and Jeff Wiese''s "Teaching in the Hospital") to colleagues hoping to improve their bedside teaching.

I must say it has helped me to teach complex conditions using frameworks that were meticulously formed by Andre.
The text is also a nice compliment to the frameworks and a great reminder of each complex condition that can contribute to the problem being entertained.
After using the E-Version of this book during rounds, I have adopted this method and have been making my own frameworks for many clinical situations that arise during rounds. Many of my learners appreciate this method of breaking down the clinical problem and are buying this book to further learn concepts laid down in this book.
This book is a must buy for faculty and especially the ones that are early in their career since it will help augment your teaching and may also give you ideas how to discuss concepts during rounds, morning report or noon conferences.
For residents, interns and students, it gives a great idea how to construct a broad differential for many problems it will help you to understand how to formulate a hypothesis for the most likely diagnosis for your patients.

Dr. Andre Martin Mansoor is a world-class physician and this is a work of art. Not only is this book spectacular, but his online lectures, YouTube channel, Instagram page, and Twitter page are legendary and to be frank, out of this world. The material is phenomenal and the way he moves and speaks is unforgettable. I honestly learned more medicine from reading this textbook than from going to 4 years of medical school. If you are a medical professional and don''t buy this textbook, you are a complete and utter fool.

This textbook is great for both students and practicing clinicians. The frameworks have such a unique layout that is easy to understand. Unlike normal medical textbooks that have exhaustive charts and lists, this books builds the framework throughout the chapter which is much easier to digest and remember. I have yet to find a textbook that can take complex topics and present them in a way that is concise (but also thorough) as well as this book does. The chapters cover just about everything you will encounter in internal med. Even as a practicing clinician, I continue to reference the frameworks when I am struggling to find the right diagnosis.

As an extra bonus - the textbook comes with a code for a smart phone app that allows you to pull up the frameworks at any time. Super handy.
